Consent Forms for Parents
Flu Vaccination Programme for 2018 - YrR to Y5.
A consent letter will be sent home with your child (if applicable) in the next few days. Completed forms need to be returned to the school office by Thursday 25th October 2018.
Vaccination will take place in school during the school day for children who have been given consent on Friday 9th November 2018.
